We assist clients of all age groups and with all forms of disability and all kinds of difficulties in managing their healthcare. We are two experienced health professionals and qualified advocates  who have undertaken patient advocacy training & now work as healthcare support workers, case managers and capacity builders for general healthcare consumers and for disabled recipients. Excellent medical and hospital care are everyone's right. If only the medical and hospital systems were user friendly and provided a level playing field for all patients. Disabled patients frequently find that their disability means they experience problems getting the healthcare they need. Disabled patients frequently have more complex and more varied health problems than the average person. Disabled patients do not have dedicated or patient advocacy services within the healthcare system as is the case for others patient populations (eg aged, mental health, paediatric etc). Excellent care is available but access and outcomes are skewed against the disabled community. 

Your best chance of optimal outcomes from your healthcare is through expert case management and that is where we come in. We are here and ready to help. We work from the cradle to the grave covering all age groups. We are by your side and on your side, always.
 
We provide support in terms of NDIS assessment, planning, and delivery of Disability-Related Healthcare Services where these supports directly relate to your significant and permanent functional impairment and assist you to improve your ability to undertake activities of daily living. We help you overcome the barriers that the healthcare systems presents.
 
We can support you through the healthcare system when your care needs are directly related to your disability.
 
This includes help going through the screening, diagnosis, treatment, and follow-up of your disability.
 
We can help streamline your disability related healthcare services and providers through expert case management: we access, navigate, coordinate, demystify and manage your medical and hospital care. 
 
Our services help build your capacity to communicate with your disability healthcare providers so you can get the information that you need to be able to make decisions about your disability. 
 
The NDIS defines personal capacity building as: Making sure people with disability and their families have the skills, resources and confidence they need to participate in the community or access the same kind of opportunities or services as other people. In order to navigate and conquer the health care system, you may need to learn patient advocacy skills. We can show you how this is done: watch and learn!
 
Capacity Building is supporting people and families to exercise their own choice and control, develop new forms of support and skills that build independence and a happy, contented life.  When it comes to your health, a vital skill will be DIY patient advocacy: a skill for life! Capacity building is not a goal in itself.  It is an approach that empowers individuals to access and create a life that is meaningful to them now and into the future.
 
At Disability Health Support we consider capacity building a demonstration of respect for peoples individual right to live their own lives.
 
We can help you streamline your disability healthcare providers and seek out the most appropriate, disability literate, professionals for you.
 
We can also support you to grow your skills to be able to set up appointments for doctor visits, medical tests and social support.
 
We also work with schools, employers, case managers, support coordinators, plan managers, lawyers and others who may have an effect on your disability related health care needs.
 
We can provide care and maintenance where the primary purpose is to provide ongoing support for you in order to maintain a level of functioning including long-term support required to achieve small incremental gains or to prevent functional decline.
 
We can also provide support on an intensive and time limited period where high complex needs or a high level risk to you occurs impacting your ability to implement your plan and access appropriate supports.

We assist you to reduce complexity in your support environment, and overcome barriers to connecting with broader systems of supports.

We will assist you when you are experiencing a health care crisis to keep you safe and to resolve barriers that threaten your wellbeing or stop you from implementing reaching your goals.  We aim to assist to ensure a consistent delivery of service and access to relevant health supports during crisis situations.

We can also develop a complex health disability service plan that focuses on how you and your disability related healthcare team will interact to resolve barriers and promote appropriate plan implementation.

What if you need someone with training to provide the support you need?

For some disability-related health events you might need someone who is trained to deliver the support you need. You might benefit from DIY patient advocacy coaching.

Can disability-related health support be delegated or out-sourced?

Yes, some disability-related health support tasks can be delegated to another worker. This means that a trained person will complete the task for you if a family member, friend or carer can’t complete the tasks.  We will fund a support worker to provide you with your disability-related health support in some circumstances.

People with disability use a range of health services.  A range of mainstream health services—such as general practitioners (GP), medical specialists, hospitals and dentists—and their care may require coordination between different health professionals. They may need assistance to manage their healthcare. They may also rely on informal care, such as that provided by family and friends, to meet or supplement their health-care needs via the services of a complex case manager.

Service accessibility is dependent on many factors. Some people with disability experience difficulties in accessing health services, such as unacceptable or lengthy waiting times, cost, inaccessibility of buildings and discrimination by health professionals. They may also experience issues caused by lack of communication between the multiple health professionals treating them. Poor healthcare results in poor health and that means you may not lead your most independent and fulfilled life. 

People aged under 65 with severe or profound core activity limitation (that is, who sometimes or always need help with activities of self-care, mobility or communication) who lived in the community are more likely to use a range of health services, particularly medical specialists. They are also more likely to see three or more health professionals for the same condition, will benefit from a health professional to coordinate their care, and routinely face difficulties caused by siloing of information between different health professionals.

Complex Case Management (Specialist Support Coordination)

What is Complex Case Management?

Similar to NDIS Support Coordination, complex case healthcare management is a type of specialist support coordination for those with more complex health needs. If you have additional high or complex needs, you may need support from specialists including psychiatrists or mental health workers, social workers or other health professionals.

Specialist Support Coordinators help by building capacity and resilience for when good healthcare, access and communication are breaking down. We know how to get healthcare back on track and working for you. We know how navigate the health system, overcome barriers and how to respond in crisis situations.

NDIS funded support, Specialist Support Coordination is designed to facilitate the access to required supports to help you live a normal and fulfilling life.
